We are seeking a skilled and compassionate Care Management Nurse to join our team on the Clinical Decisions Unit (CDU) at South Tees Hospitals NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care to acutely unwell medical patients and supporting the delivery of cardiac monitoring services across the site.
Main Responsibilities- Assess, implement, and evaluate clinical care for patients to meet their needs in a safe and caring environment.
- Develop knowledge and skills within the specialty and assist in the management and organization of nursing work in the ward/department.
- Supervise the work of healthcare assistants and support workers and support the training and assessment of student nurses.
- Demonstrate the trust Values and Behaviours for nursing and midwifery staff at all times.
- Comply fully with best practice standards and trust policies for personal and patient safety and the prevention of healthcare-associated infections.
