About the Role

We are seeking a highly skilled and experienced Primary Care Nurse Educator to join our Enhanced Care Home Team. As a key member of our team, you will play a vital role in providing leadership, education, and support to care homes, with a focus on reducing unnecessary hospital admissions and improving patient outcomes.

Main Responsibilities
  • Develop and deliver educational packages and advice to care homes on best practice and individual needs
  • Assist in addressing specific patient needs, including review, advice, or signposting to other services as appropriate
  • Work with all disciplines and professionals to educate, develop, and support clear understandings of patient needs and management within the care home environment

Person Specification
  • Registered Nurse with current NMC registration
  • Evidence of continuing professional education and training at degree level
  • Post-registration qualification in relevant field at specialist level or equivalent experience
  • Recognised teaching qualification or equivalent experience as part of role
  • Member of special interest group
  • Prescriber
Knowledge and Training
  • Knowledge and application of principles of care for enhanced care homes, long-term conditions, relevant NSFs, and other national standards
  • Knowledge and awareness of NHS issues, current healthcare policy, and relevance to the delivery of healthcare services
  • Knowledge of common clinical presentations in the primary care setting and application of successful treatment plans and outcomes
  • Knowledge and application of NMC or relevant body Code of Professional Conduct
  • Knowledge and application of Personal Development Planning
  • Detailed knowledge of Primary Care and collaborative working with other care providers, e.g., Care Homes, Social Care, and Voluntary services
  • Knowledge and application of evidence-based practice
  • Knowledge and application of clinical governance
  • Knowledge and application of managing difficult situations and conflict resolution
  • Knowledge and application of how equal opportunities can be implemented in practice
  • Ability to work under pressure and meet deadlines

  • Experience of working in collaboration with and across organisational boundaries

  • Evidence of knowledge in caseload management
  • Experience in assessments for deteriorating patients and developing/delivering care plans to meet these needs
  • Experience of teaching and mentoring students
  • Experience of service improvement
  • Experience and evidence of recent post-registration experience in Community Rapid Response/Urgent Care and Discharge planning
  • Experience in audit and/or research
