Adult Mental Health Practitioner

We are seeking a highly skilled and dedicated Adult Mental Health Senior Practitioner to join our team at Pennine Care NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality mental health services to our patients.
Key Responsibilities- Conduct thorough psycho-social assessments of clients, including risk assessments, safety plans, and care plans.
- Deliver individualized interventions to clients based on their care plans.
- Liaise with local services, multidisciplinary teams, and third-sector organizations to ensure seamless care.
- Manage a complex community caseload and provide support to clients with drug and/or alcohol problems.
- Short course and experience to post-graduate diploma level.
- Knowledge of Primary Care Mental Health and delivery of brief interventions.
- Certificate/diploma in CBT/counselling or CPD in Primary Care Mental Health.
