About the Role

We are seeking a skilled and empathetic Community Health Worker to join our Alcohol Service Team. This is an excellent opportunity to work with individuals affected by alcohol misuse and contribute to their recovery journey.

Job Description

The successful candidate will work collaboratively with our team to provide comprehensive support to patients struggling with alcohol-related issues. This includes supporting earlier discharge from hospital or admission avoidance, assisting service users with complex physical health needs, and delivering targeted interventions to promote positive change.

Key Responsibilities
  • Communicate effectively with service-users, carers, colleagues, and external agencies using various methods, including Information Technology.
  • Regularly report and communicate service-users' health status and care aims in Halo System and provide verbal updates to Registered Health Professionals and other team members.
  • Relate appropriately with service-users, carers, colleagues, and others, maintaining professional boundaries.
Requirements
  • Advanced Apprenticeship in Mental Health and Social Care or NVQ 3 in substance misuse with demonstrable evidence of numeracy and literacy skills (equivalent GCSE's or Functional Skills).
  • Experience working with people with substance misuse needs as a member of a multi-disciplinary team.
