We are excited to announce an opportunity for a skilled Community Nursing Team Member to join our team at Kent Community Health NHS Foundation Trust. As a key member of our community nursing team, you will play a vital role in delivering high-quality patient care and support.

Job Description

The successful candidate will be responsible for providing community-based nursing care to patients in their homes, residential care homes, and other settings. This will involve working closely with healthcare professionals to develop and implement care plans that meet the individual needs of our patients.

Key Responsibilities

  • Providing community-based nursing care to patients in their homes, residential care homes, and other settings
  • Developing and implementing care plans that meet the individual needs of patients
  • Working closely with healthcare professionals to ensure seamless delivery of care
  • Maintaining accurate records and reporting any changes or concerns to relevant staff members

Requirements

  • A minimum of 2 years' experience as a Healthcare Assistant/ Worker in a health or social care setting
  • Relevant level 3 qualification (NVQ/ QCF Diploma or certificate of care / Advanced apprenticeship) or equivalent
  • Excellent communication and interpersonal skills
  • A valid driving licence and access to a car for work purposes

Salary and Benefits

  • A competitive salary range of £24,907 - £30,615 per annum
