About the Role

This is a Monday to Friday position that is not part of a rotating schedule. As a Sustainable Recovery Specialist, you will provide support to individuals before and after their program to help them achieve their goals and improve their lives. You will be part of a dedicated and skilled team, contributing to the delivery of tailored interventions and recovery approaches.

To ensure residents receive ongoing support after completing residential and housing services, we will implement a continuum of care approach to assist individuals in sustaining their recovery.

We are seeking a professional to lead the delivery of systematic reviews and support sessions, as well as drop-in sessions (virtual and in-person), providing responsive support on a one-to-one and group basis.

Support the development of national residential rehabilitation aftercare support packages to promote a continuum of care.

About You

To be successful as a Sustainable Recovery Specialist at Phoenix Futures, you will need:

  • Relevant experience working in health and social care, preferably in the field of substance use, mental health, or a comparable field
  • Evidence of effective partnership working and the ability to network for the benefit of the client and service
  • Evidence of managing a busy caseload or tasks and excellent organizational skills
  • Proficiency in the use of Microsoft Outlook and Word and familiarity with database management systems
  • Excellent communication skills, both written and verbal
  • Determination, with a drive to succeed and a willingness to learn
  • Passion and enthusiasm for making a real difference in the lives of the individuals we support
