Main Responsibilities:

  1. Develop Comprehensive Care Plans: Work closely with the multidisciplinary team to create tailored care plans that address the unique needs of each patient.
  2. Collaborate with Healthcare Professionals: Collaborate with other healthcare professionals to develop and refine care strategies that meet the complex needs of patients.
  3. Clinical Risk Management: Identify potential risks and implement effective management strategies to ensure high-quality patient care.

Requirements of the Role:

  • Registered Mental Health Nurse (RMN) or Learning Disability Nurse (RNLD) with active NMC registration and specialised training in relevant clinical skills.
  • A strong clinical leader with experience in leading service development projects and working across organisational boundaries.
