About the Role

We are seeking a highly skilled and experienced Senior Community Mental Health Practitioner to join our established team at Havant and Waterlooville OPMH Team on a fixed term contract.

The successful candidate will work as a core member of the Community Mental Health team, coordinating the care of a complex caseload of service users and supervising a Band 5 Community Practitioner/Band 3 Community HCSW where appropriate.

Working under the supervision of the Deputy Team Manager/Team Manager, the practitioner will formulate care and risk management plans in accordance with identified need, ensuring they are recovery focused and developed in collaboration with the service user and where appropriate their relative and carer.

Main Duties
  • Coordinating the care of a complex caseload of service users
  • Supervising a Band 5 Community Practitioner/Band 3 Community HCSW where appropriate
  • Formulating care and risk management plans in accordance with identified need
  • Ensuring care plans are recovery focused and developed in collaboration with the service user and where appropriate their relative and carer

Person Specification
  • Registered Nurse, Mental Health Practitioner or Occupational Therapist
  • Recognised Mentorship qualification (applies to nurses) or preparedness to undertake this within agreed timeframe
