About the Role

We are seeking a highly skilled and experienced Senior Community Mental Health Practitioner to join our established team at Havant and Waterlooville OPMH Team on a fixed term contract.

The successful candidate will work as a core member of the Community Mental Health team to co-ordinate the care of a complex caseload of service users, supervising a Band 5 Community Practitioner/Band 3 Community HCSW where appropriate.

Working under the supervision of the Deputy Team Manager/Team Manager, the practitioner will formulate care and risk management plans in accordance with identified need and ensure that these are recovery focussed and developed in collaboration with the service user and where appropriate their relative and carer.

Main Responsibilities
  • Co-ordinate the care of a complex caseload of service users
  • Supervise a Band 5 Community Practitioner/Band 3 Community HCSW where appropriate
  • Formulate care and risk management plans in accordance with identified need
  • Ensure that care and risk management plans are recovery focussed and developed in collaboration with the service user and where appropriate their relative and carer

Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.

  • Registered Nurse, Mental Health Practitioner or Occupational Therapist
  • Recognised Mentorship qualification (applies to nurses) or preparedness to undertake this within agreed timeframe
