Community Healthcare Practitioner Role

We are looking for an experienced Community Healthcare Practitioner to join our team at Communication Partners. This dynamic role will involve working in a fast-paced community setting, providing high-quality care to patients in their homes.

As a Community Healthcare Practitioner, you will be responsible for delivering safe, effective, and person-centred care to patients who require community nursing care. You will work collaboratively with our community nursing team to develop and implement individualised care plans, using your knowledge and skills to promote optimal patient outcomes.

Key Responsibilities:

  • Conducting comprehensive assessments of patients' needs and developing tailored care plans
  • Delivering clinical and administrative duties in a timely and efficient manner
  • Communicating effectively with patients, carers, and other healthcare professionals to ensure seamless care coordination
  • Participating in ongoing training and development to maintain and enhance professional skills
