We are looking for an experienced and caring Support Worker to join our team in Perth. As a Support Worker, you will play a vital role in supporting vulnerable individuals to live independently and achieve their goals.

About the Role

As a Support Worker, you will be responsible for providing individualised support to service users, promoting their independence and well-being. You will work collaboratively with colleagues to ensure that each person receives the care and support they need.

The ideal candidate will have excellent communication and interpersonal skills, with the ability to build strong relationships with service users, colleagues, and external partners. A basic understanding of health and social care principles would be beneficial.

Key Responsibilities
  • Providing individualised support to service users, including personal care, medication management, and daily living tasks.
  • Assisting service users with activities and outings, promoting their independence and participation in community life.
  • Maintaining accurate records of service user interactions, using a tablet-based system.
Requirements

To be successful in this role, you will require:

  • A friendly and approachable demeanor, with excellent communication and interpersonal skills.
  • Basic knowledge of health and social care principles, with a willingness to learn and develop your skills.
  • The ability to work collaboratively as part of a team, prioritising the needs of service users and colleagues.
