Care Coordinator

Job Summary:
At PP Associates, we are seeking a dedicated and experienced Adult Social Care Specialist to join our team. The successful candidate will be responsible for assessing, planning, and implementing support for adults with varying needs, ensuring they receive the highest quality of care and support.
Key Responsibilities:
- Conduct thorough needs assessments of individuals, considering physical, emotional, and social factors.
- Develop and implement tailored care plans in collaboration with clients and their families.
- Advocate for clients' rights and ensure their voices are heard in decision-making processes.
- Provide guidance and support to clients facing complex challenges, including mental health issues, disabilities, and social isolation.
- Collaborate with multi-disciplinary teams to ensure integrated service delivery.
- Regularly review and update care plans to meet changing needs.
- Maintain accurate and up-to-date case records in accordance with legal and organisational standards.
- Participate in supervision, training, and professional development to enhance skills and knowledge.
- Contribute to the development of policies and procedures related to adult social care services.
Qualifications:
- A recognised Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
