We are seeking two dedicated Hospital Link Workers to join our Community Diversion Service in Northumberland. As a Hospital Link Worker, you will play a pivotal role in supporting individuals who present with injuries related to serious violent crime. Your primary responsibility will be to provide comprehensive support to these individuals, helping them access our Community Diversion Service and motivating them to make positive changes in their lives.

Key Responsibilities
  • Manage a caseload of service users, providing tailored support to meet their unique needs.
  • Develop and implement ongoing support plans and treatment strategies in collaboration with service users.
  • Work closely with a diverse team of professionals to ensure a holistic and integrated approach to support.
  • Assist service users in setting and working towards their goals, fostering a safe and supportive environment.
Requirements
  • A strong commitment to the well-being and rehabilitation of service users.
  • Experience in substance misuse, support work, mental health, or criminal justice.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively within a multidisciplinary team.
  • Knowledge of best practices in safeguarding children and vulnerable adults.
