Main Responsibilities
- Provide expert support to our practices on medication and prescription queries, ensuring patients receive the best possible care.
- Work closely with our team to develop and implement effective medication management strategies, including repeat prescription processes and chronic disease management.
- Conduct thorough medication reviews, proactively managing patients with complex polypharmacy and ensuring safe and effective prescribing practices.
- Collaborate with our team to manage patients with long-term conditions, providing personalized care and support.
- Manage medication reviews and safe prescribing practices in care homes, ensuring the highest standards of care.
- Provide exceptional patient-facing and telephone support, addressing patient queries and concerns regarding their medication.
- Stay up-to-date with the latest MHRA alerts, local, and national guidance, implementing changes as required.
- Offer expert advice and recommendations on shared care, NICE guidance, and RAG lists, ensuring our team is equipped to provide the best possible care.
- Manage common and minor ailments, working within the limits of your capability and ensuring the highest standards of care.
- Oversee discharge summaries from hospitals and community services, ensuring seamless care transitions.
