Job Summary

We are seeking a skilled Clinical Pharmacist to join our team at North Watford Primary Care Network. As a Clinical Pharmacist, you will play a vital role in supporting our practices with medication and prescription queries, ensuring the safe and effective prescribing of medications in care homes, and providing patient-facing and telephone support to patients with queries and concerns regarding their medication.

Main Responsibilities

  • Provide support to practices with medication and prescription queries.
  • Support the practices in chronic disease management, medication reviews, and proactively managing patients with complex polypharmacy.
  • Be actively involved in the management of patients with long-term conditions.
  • Manage medication reviews and safe and effective prescribing in care homes.
  • Provide patient-facing and telephone support to patients with queries and concerns regarding their medication.
  • Implement changes required regarding MHRA alerts and local and national guidance.
  • Provide advice and recommendations on shared care, NICE guidance, and RAG lists.
  • Manage common and minor ailments while working within the limits of your capability.
  • Manage discharge summaries from hospitals and community services.

Person Specification

Qualifications

Essential

  • Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
  • Minimum of 2 years' experience as a pharmacist, demonstrated with a practice portfolio.

Experience

  • Experience and awareness of common acute and long-term conditions that are likely to be seen in general practice.
  • Ideally hold or be working towards an independent prescribing qualification.
  • Able to follow legal, ethical, professional, and organisational policies/procedures and codes of conduct.
