Job Overview
We have exciting opportunities for Specialist Community Public Health Nurses to join our integrated 0-19/25 neighbourhood teams. As part of our team, you will work as an autonomous practitioner, leading on the delivery of The Healthy Child Programme.
This programme provides every child with a schedule of health and development reviews, health promotion, screening, and support for parents, carers, children and young people tailored to their needs. Universal health and development reviews are key features of the programme, assessing children, young people and families' strengths, needs and risks.
You will also be responsible for providing additional support at key times to protect children, young people and families who are experiencing harm or are at risk of suffering significant harm. This supports a model of progressive universalism; a core universal programme for all children with additional contacts for those with identified need.
Key Responsibilities
- Deliver an evidence-based service using a wide range of assessment and intervention tools.
- Work collaboratively with other agencies to ensure protection and the best health outcomes for children and families in Bolton.
- Manage a universal plus and partnership plus caseload.
- Support the delivery of the universal offer in a 0-19 integrated neighbourhood team.
Benefits

The estimated annual salary for this position is £41,000 - £53,000 depending on experience.
