Children's Community Nurse

About the Role
We are seeking a highly skilled and experienced Children's Community Nurse to join our team. The successful candidate will be responsible for providing high-quality care to children and young people with complex and life-limiting conditions.
Key Responsibilities
- Assess and plan the care needs of children and young people, taking into account their physical, psychological, and social needs.
- Develop and implement individualized care plans, working closely with families and other healthcare professionals.
- Provide education and support to families, carers, and other healthcare professionals to promote the best possible outcomes for children and young people.
- Collaborate with other healthcare professionals to ensure seamless care transitions and effective communication.
- Participate in multidisciplinary team meetings and contribute to the development of care plans and policies.
Requirements
- Registered Sick Children's Nurse/Child Branch qualification.
- 2 years post-registration experience in a similar role.
- Experience of working within a multi-disciplinary team and utilizing IT skills to document events and disseminate information.
- Ability to supervise junior staff and provide 1:1 and appraisal monitoring.
- Dexterity and coordination required for IV, CVL, IM interventions, and administration of medications.
- Knowledge of relevant policies and procedures, including safeguarding and disability.
- Knowledge of recognized health-related issues in children and young people with complex and life-limiting conditions.
