Physiotherapist for Complex Care Team

We are seeking a highly skilled Physiotherapist to join our Complex Case Team, supporting patients in care homes across Bexley. The successful candidate will work closely with our multidisciplinary team to provide interventions and support residents in avoiding unnecessary hospital admissions.
The Physiotherapist will be responsible for assessing and managing patients who have experienced falls, working closely with care home staff to develop strategies for prevention and management. They will also support the Occupational Therapist with falls training, ensuring care home staff are up to date with relevant requirements.
