We are seeking a skilled Specialist Physiotherapist to join our team at the Central London Community Health Trust. As a key member of our Discharge to Assess Team, you will play a vital role in delivering high-quality reablement services to Brent residents.
In this exciting opportunity, you will work closely with Adult Social Care colleagues and Reablement teams to provide world-class care in the community. Your expertise as a physiotherapist will enable you to develop and implement treatment plans for clients with complex needs, including neurological conditions, musculoskeletal issues, and more.

Key Responsibilities:
- Undertake specialized assessments and develop treatment plans for clients with complex conditions

Requirements:
- Degree in Physiotherapy from an accredited institution
- Current registration with the HCPC and evidence of ongoing CPD
- Experience working with adult populations, particularly in rehabilitation settings
- Strong clinical reasoning and problem-solving skills, with ability to adapt to changing situations
