Job Summary

Castle Partnership is seeking a skilled Advanced Practitioner to join our team in Central Norwich. As a key member of our healthcare team, you will provide high-quality patient care to those who are unable to be seen in our practices for various reasons.

Main Responsibilities

As an Advanced Practitioner, you will be responsible for:

  • Conducting home visits to assess, diagnose, and treat patients with acute presentations of illness.
  • Diagnosing and treating patients in their own homes.
  • Liaising with our practice team to ensure referrals and signposting to relevant agencies for patients.
  • Attending clinical governance and multidisciplinary team meetings within our practices.
  • Supporting with long-term condition and chronic disease management, if of interest.
  • Identifying areas of health inequalities and communicating this to our practices to enable service improvement.
  • Liaising with GPs regarding patients who may need to be moved to a palliative pathway to enable care transfer.
  • Working as part of a team, both the practice teams and the Central Norwich Home Visiting Team.
  • Supporting with seasonal or unprecedented needs, such as vaccine delivery, if required.

Requirements

To be successful in this role, you will need:

  • Qualified Paramedic Practitioner or Nurse Practitioner.
  • General practice/community experience.
  • A full UK driving license.
  • The ability to work as a team or alone.
