Role Overview: As a Lymphoedema Care Coordinator, you will play a crucial role in ensuring the smooth delivery of care to patients with lymphoedema.

Key Responsibilities:

  • To coordinate the care of patients with lymphoedema, working closely with other healthcare professionals to ensure effective management of their condition.
  • To communicate effectively with patients, families, and other stakeholders regarding care plans and progress.
  • To maintain accurate records and reports, ensuring that data is up-to-date and reliable.

Requirements:

  • Registration with the HCPC as a nurse practitioner.
  • Proven experience in a similar role or setting.
  • Able to work effectively within a fast-paced environment.

Salary Range: The estimated salary range for this role is £35,000 - £45,000 per annum.
