Job Description

Changemh's Community Link Worker position involves supporting individuals affected by mental health or mental welfare issues in Foyers. The successful candidate will work closely with healthcare professionals to identify areas of need and develop personalized support plans.

The role requires collaboration with statutory and non-statutory agencies to ensure comprehensive care. The ideal candidate will have educated to SVQ level 2 or relevant experience in health and social care.

Responsibilities and Qualifications
