As a Community Nurse, you will be responsible for delivering high-quality nursing care to patients in their own homes or community settings, supporting them to maintain independence and achieve their healthcare goals.

With a strong clinical background and excellent communication skills, you will work collaboratively with other healthcare professionals to ensure coordinated care. You will conduct holistic assessments of patients' physical, emotional, and social needs, develop individualised care plans, and monitor progress to achieve the best outcomes.

The ideal candidate will have experience in a relevant setting, such as community, primary care, or acute services, and possess strong clinical assessment and decision-making skills. A full UK driving licence and access to a vehicle for work purposes are also essential.

This role offers a competitive salary of £32,000 - £43,500 per annum, depending on experience, plus benefits including flexible working hours, 24/7 support from your own dedicated consultant, and access to CPD and career development pathways.
