The Lewisham Care Partnership is seeking a skilled and compassionate Care Navigator to join our team. This role involves working as the first point of contact for patients, providing excellent customer service, and arranging appointments with healthcare professionals.

Responsibilities

  • Process patient requests for appointments and signpost them to the appropriate healthcare professional.">
  • Answer incoming phone calls, transfer calls, or deal with callers' requests appropriately.">
  • Enter read-code data on the Emis Web clinical system.">
  • Carry out system searches as requested.">

Requirements

">

This role requires a GCSE level or equivalent qualification, active signposting or care navigation training, and experience working in a primary care environment.

A strong IT background, including EMIS / Systmone / Vision user skills, is also essential.

Salary and Benefits

">

We offer a competitive salary range of £12.00-£12.48 per hour, with benefits including company pension, employee discount, and free flu jabs.

Working Hours

This position is part-time, with an estimated 20-37.5 hours per week.

This role will be based at The Lewisham Care Partnership's central call Centre and practices sites within the central Lewisham area.
