About the Role

We are seeking an experienced and skilled Registered Nurse to join our Continuing Healthcare (CHC) Team as a Lead Nurse for the Physical Disability CHC Team. The successful candidate will have a strong understanding of the CHC legislation and process, as well as experience working in the physical disability field of practice.

Key Responsibilities
  • Undertake CHC assessments and reviews for patients across all aspects of care, requiring a high level of assessment skills and effective communication with colleagues, healthcare professionals, service users, carers, and family members.
  • Develop and maintain effective communication networks with health and social care professionals and act as a resource to them.
  • Work with relevant health professionals, CHC colleagues, business support staff, senior leads, and other support staff within the CHC team to ensure a seamless and efficient service delivery.
  • Provide specialist nursing advice to secondary care, primary care, nursing homes, etc., to facilitate the continuing healthcare process.
  • Ensure timely end-of-life provision and reviews, following national guidance for clients fast-tracked through the CHC system.
  • Deliver formal and non-formal education programs to community and hospital staff, including nursing/residential homes, GPs, community nursing, and therapy staff, relating to the assessment and management of CHC patient groups in line with the latest research, evidence, and Department of Health/NICE guidance.
Person Specification
  • Registered Nurse (RGN/RMN/RNLD/RSCN)
  • Educated to degree level or equivalent in experience
  • Current and relevant NMC registration
  • Post-graduate qualification or specialist training, i.e., Community Nursing
