About the Role

As a highly skilled and motivated registered nurse, you will be responsible for providing expert direction and leadership to a team of community nurses. You will work clinically to offer assessment and nursing care, advice, and information to patients and carers within the home environment or alternative care settings.

Main Responsibilities
  • Maintain patients in their preferred place of care, enabling them to maximise their independence and optimise their quality of life.
  • Facilitate timely discharge from hospital, including those patients at End of Life.
