Care Manager for Adults with Complex Needs

We are seeking an experienced Care Manager to join our team at Devon Partnership NHS Trust. As a Care Manager, you will be responsible for assessing the needs of adults with complex health requirements and commissioning packages of care that meet their individual needs.
As part of this role, you will work closely with multi-disciplinary teams to ensure that patients receive high-quality, coordinated care. You will also be responsible for reviewing eligibility for Continuing Healthcare Funding and determining ongoing eligibility for CHC / FNC.
The ideal candidate will have experience working with adults with learning disabilities and a sound understanding of the Continuing Healthcare Framework, including Funded Nursing Care and Section 117 aftercare.
We offer a competitive salary of £40,000 - £55,000 per annum, depending on experience, and a range of benefits, including a relocation package of up to £8k available.
