About the Role

We are seeking a skilled and compassionate Health Improvement Practitioner to join our team at Tāmaki Health. As a key member of our Wellness Support Team, you will play a vital role in providing holistic support to individuals and whānau with psychosocial, mental, physical, and wellbeing needs.

Key Responsibilities
  • Provide brief behavioural and psychological interventions to support mental and physical health and wellbeing for our clinic populations.
  • Work alongside multi-cultural GP teams, health coaches, and community support workers in a collaborative and integrated manner.
  • Support nurses and GPs to promote well-being in routine GP care.
  • Link patients with other support services available through the Wellness Support Team in a stepped model of care.
  • Contribute to the development and growth of this service.
Requirements
  • A post-graduate qualification enabling you to work therapeutically with clients.
  • Registration with a registering body under the Health Practitioners Competence Assurance Act (HPCA).
  • Strong cultural and linguistic abilities to work in our diverse Tāmaki Makaurau communities.
