Job Title: Community Mental Health Nurse

Midlands Partnership NHS Foundation Trust is seeking a skilled Community Mental Health Nurse to join our team.

Job Summary:

We are looking for a dedicated and compassionate Community Mental Health Nurse to work as a senior member of our Early Intervention in Psychosis Service. The successful candidate will support the organisation in effectively delivering Nice Concordant Early Intervention in Psychosis Service to all appropriate referrals.

Key Responsibilities:
  • Provide specialist Early Intervention consultation/liaison and advice to colleagues.
  • Contribute and undertake specialist Early Intervention assessments for those referred to the service with suspected psychosis and/or At Risk Mental State.
  • Plan, implement, and evaluate evidence-based interventions required, including health promotion for a named service user/group under the guidance of the team leader/senior practitioners.
  • Responsible for the appropriate clinical risk assessment of Early Intervention service users, contacting the Out of Hours services where required, providing effective advice, guidance, and decision-making process culminating in the coordination of any reviewed plan of care under the supervision of the team leader/senior practitioners.
  • Organise admissions, case conference/reviews, and discharge planning in collaboration with members of the multi-disciplinary team under the supervision of the pathway lead/senior practitioners.
  • Coordinate the care and assessment under the Community Mental Health Framework of a designated caseload under the supervision of the pathway lead/senior practitioners.
  • Responsible for the safe custody and administration of medicines where applicable to their profession.
  • Ensure that the agreed Early Intervention in Psychosis philosophy approach is adopted and followed, whilst respecting and valuing the different therapeutic approaches available within the multi-disciplinary team.
  • Maintain confidentiality at all times in accordance with the data protection act, trust policy, and good practice.
  • Make autonomous decisions with recourse to supervision in relation to complex social care needs and risk in relation to individuals.
Requirements:

The ideal candidate will have a strong background in mental health nursing and experience working with individuals with psychosis and/or At Risk Mental State. They will be able to work effectively as part of a multi-disciplinary team and have excellent communication and interpersonal skills.

The successful candidate will be required to work in a fast-paced environment and be able to manage competing priorities. They will also be required to maintain accurate records and contribute to the development of the service.
