About the Role

We are seeking a qualified and experienced Mental Health Liaison Practitioner to join our team at Midlands Partnership NHS Foundation Trust. As a key member of our team, you will be responsible for providing comprehensive mental health assessments and direct delivery of specialist clinical care to patients in acute and community hospital settings.

Key Responsibilities
  • Assess the inter-relationship of physical illness, psychological, and psychosomatic presentations.
  • Develop and monitor specialized mental health programs of care.
  • Provide highly specialized advice across client groups, including young people, adults of working age, people over 65, and those experiencing substance misuse problems.
  • Deliver evidence-based therapeutic interventions, including complex cases with high risk following episodes of self-harm and attempted suicide.
  • Support the education, advice, and support of Shrewsbury and Telford Hospital staff in their care of individuals with mental health presentations.
Person Specification
  • UK registration as a qualified mental health professional.
  • Experience of mental health assessment and direct delivery of specialist clinical care.
  • Ability to work independently and as part of a team.
  • Positive attitude and commitment to delivering high-quality patient care.
