Mental Health Practitioner Development Post: A Career-Defining Opportunity

This is a developmental post suitable for Band 5 Nurses or Occupational Health Practitioners looking for a supported path to a B6 role.
As a Mental Health Practitioner, you will work in a multi-disciplinary team and in partnership with internal and external agencies to provide specialist assessment and care planning in conjunction with service users and carers.
You will evaluate treatment plans and undertake home treatment, triage and assess referrals received by the service in liaison with multi-disciplinary colleagues, and co-ordinate the work of the Team.
