About the Role

We are seeking a highly skilled and dedicated District Nurse / Community Sister/Charge Nurse to join our PCN Community Team on a 9-month Fixed Term contract. As a key member of our team, you will work in partnership with individuals and their carers to develop a personalized plan of care, proactively monitoring and anticipating the changing needs of these individuals, and coordinating their care across all parts of the health and social care system.

Key Responsibilities
  • To apply advanced assessment of physical, mental, and social care needs to provide clinical care and health-promoting interventions that help avoid unnecessary hospital admission.
  • To interpret clinical signs and symptoms and diagnostic tests in order to develop a treatment plan and/or treatment in conjunction with medical practitioners as appropriate. Provide advanced clinical advice to the wider healthcare team to improve outcomes of patient care.
  • Ability to develop and analyze audit, interpret data, and use findings to enhance clinical practice.

Qualifications

  • NMC Registered Nurse
  • Post-Registration study or qualification
  • Mentorship or equivalent / practice assessor
  • Leadership/Management course or willing to work towards.
  • Significant experience of working within a community setting.

Other Role Requirements

  • Able to work in a flexible way and respond positively to change.
  • Ability to travel between sites, car owner/driver, access to vehicle for work purposes.
