We are seeking a skilled Support Worker to join our team at Platfform in Neath Port Talbot. The successful candidate will be responsible for providing social, emotional, and practical support to individuals experiencing mental health challenges.

This role is focused on ensuring the stability of tenancies across the area, with a strong emphasis on aligning our support services with Platfform's values.

The working hours will be Monday to Friday, 9 am – 5 pm, with an optional on-call telephone service covering evenings from 5 pm – 9 pm. This will require staff to cover the service if needed, with an additional allowance provided.

Key Responsibilities:

  • To provide holistic support to individuals facing mental health issues that may impact their ability to maintain stable tenancies.
  • To work closely with clients to understand their needs and develop tailored support plans.
  • To collaborate with internal teams to ensure seamless delivery of support services.
  • To contribute to the development and implementation of strategies to enhance client outcomes.
  • To participate in training and professional development opportunities to enhance skills and knowledge.

Requirements:

  • A degree in a relevant field or equivalent experience in a related sector.
  • Proven experience in providing support services, particularly in mental health settings.
  • Excellent communication and interpersonal skills.
  • Able to work independently and as part of a team.
  • Flexibility to adapt to changing priorities and deadlines.
