About the Role

The Community Health Care Assistant will work autonomously and as part of a multi-disciplinary team to provide high-quality care to patients in their own homes. The role involves working to care plans, protocols, and policies, while reflecting individual choice and maximizing patients' independence and quality of life.

Main Responsibilities

  • Maintain the dignity and respect of patients at all times.
  • Provide planned advice, support, and instruction to patients and their carers to promote understanding of treatment plans and goals.
  • Provide domiciliary care for patients who are unable to do so, encouraging them to reach their optimum level of independence.
  • Maintain excellent communication with patients, relatives, and members of the MDT regarding all aspects of care.
  • Undertake manual handling and therapeutic handling techniques with patients, both single-handed and as part of a team.
