Child Health Support Worker

We are seeking an experienced and caring Paediatric Nurse to fill a key role within our organisation. As a Complex Case Nurse, you will be responsible for managing complex care packages, providing specialist care and support to vulnerable individuals, and collaborating with healthcare professionals and community teams.
Key Duties:- Support safe transition and mobilisation of complex packages from hospital to home.
- Maintain services for existing clients, ensuring ongoing high standards of care and safety.
- Work collaboratively with Complex Care Managers (CCMs), ICBs, and other professional bodies to coordinate new client packages and ensure timely remediations if required.
- At least 5 years of experience working in paediatric care or a related field.
- Proven ability to manage complex cases and provide specialist care.
- Strong communication and interpersonal skills.
- Valid UK driving license and access to a reliable vehicle.
