Hunter Gatherer Mental Health is seeking a skilled Mental Health Practitioner to join their Adult team in Greater Manchester. As a key member of the team, you will be responsible for providing high-quality patient care and working collaboratively with colleagues to achieve excellent patient outcomes.

Key Responsibilities
  • Provide assessments and prioritize appointments for individuals with acute mental health needs.
  • Develop and implement effective management plans, follow-up care, referrals, and fast-tracking of patients into specialist services or to Local Liaison and Welfare.
  • Deliver brief crisis interventions and follow-up appointments to support crisis management and harm minimization for a time-limited period.
  • Collaborate with multidisciplinary teams to provide comprehensive care and support to patients.
Requirements
  • Current and valid registration with the NMC.
  • Proven experience within a CPN recovery role.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Commitment to providing high-quality patient care.
