Key Responsibilities

The successful candidate for this role as an Adults Health and Care Professional will be responsible for conducting community assessments after hospital discharge, focusing on the rehabilitation and recovery of adults in need. You will work collaboratively with clients, healthcare professionals, and family members to ensure seamless care and support.

Essential Qualifications:
  • Bachelor's Degree in Occupational Therapy or equivalent.
  • Registration with the Health and Care Professions Council (HCPC) as an Occupational Therapist.
Benefits and Compensation:

We offer a competitive salary of £45,000 - £60,000 per annum, dependent on experience. The selected candidate will be expected to adhere to our equal opportunities policy.
