About the Role

We are seeking a highly skilled and compassionate Hospice Community Nurse to join our team. As a key member of our multidisciplinary team, you will provide high-quality care and support to patients and their families in their own homes, outpatient clinics, and wellbeing groups.

The successful candidate will have a strong background in palliative care, with experience in symptom control, emotional support, and guidance around social concerns. You will work closely with our medical team, AHPs, and family support to deliver comprehensive care and support to our patients.

Key Responsibilities
  • Provide high-quality care and support to patients and their families in their own homes, outpatient clinics, and wellbeing groups.
  • Work closely with our medical team, AHPs, and family support to deliver comprehensive care and support to our patients.
  • Develop and implement care plans that meet the individual needs of our patients.
  • Provide emotional support and guidance to patients and their families.
  • Work autonomously and as part of a team to make decisions that will have a positive impact on the lives of our patients and their families.
Person Specification
  • Registered General Nurse (Level 1) / Paramedic
  • Evidence of Continuing Professional Development
  • Palliative care experience is essential
  • Car owner and driver
  • Ability to perform assessment, planning, implementation, and evaluation of care
  • Good team player who is willing to support others and learn new skills
  • Flexible working to defined shift patterns including weekend working
