Care Coordinator Specialist

We're seeking a compassionate Care Coordinator Specialist to join our team at Creative Support. In this role, you'll play a vital part in enabling individuals with physical disabilities and complex health needs to live fulfilling lives.

As a Care Coordinator Specialist, your key responsibilities will include:
  • Providing person-centered care to 10 individuals across 2 services.
  • Coordinating and supervising a dedicated team to ensure the highest quality of care and support.
  • Collaborating closely with families, professionals, and stakeholders to create positive outcomes.
  • Empowering service users to engage in their communities, pursue personal goals, and maintain their health and wellbeing.
  • Supporting day-to-day operations, including deputising for the Registered Manager when required.

Our ideal candidate will have a strong passion for promoting independence, dignity, and a positive lifestyle for others. We offer a flexible work schedule, with 37.5 hours per week on a rota, including evenings, weekends, public holidays, and sleep-in shifts.
