Job Summary:

We are seeking a highly skilled and compassionate Mental Health Support Worker to join our team at Gray Healthcare. As a Mental Health Support Worker, you will play a vital role in supporting an individual with Organic Personality Disorder and dysarthria as a result of brain injury, who is living independently in their own home with 2:1 support 24 hours a day.

Key Responsibilities:

  • Provide high-quality support to the individual, promoting their independence and well-being.
  • Assist with daily living tasks, such as personal care, meal preparation, and household chores.
  • Support the individual in accessing community resources and engaging in activities that promote their physical and mental health.
  • Collaborate with the multidisciplinary team to develop and implement a personalized care plan.
  • Monitor and report any changes in the individual's condition, ensuring their safety and well-being.

Requirements:

  • Full UK Driving License - Essential.
  • Social care experience, ideally within mental health and with some experience of supporting individuals with Schizophrenia.
  • Experience providing encouragement to a person with complex health needs and Learning Disabilities.
  • Experience supporting individuals with behaviours which may be considered challenging.
  • Experience implementing detailed care plans and risk assessments.
  • Current Right to Work in the UK and have been employed in the UK for at least 6 months.
  • Enhanced DBS check and satisfactory references.
