Job DescriptionRole Summary

This role involves working as part of a multidisciplinary team, splitting time between face-to-face clinical reviews with patients and desk-based activities according to the needs of the practice.

The post holder will work within their professional boundaries, supporting and collaborating with pharmacists in general practice to provide expert clinical medicines advice while addressing both public and social care needs of patients in GP practice(s).

Main Responsibilities:
  • Support and manage the repeat prescription system, handle acute prescription requests, and reconcile medicines on transfer of care and systems for safer prescribing.
  • Signpost to community pharmacy and refer to GPs or other healthcare professionals where necessary.
  • Provide a telephone help line for patients with questions, queries, and concerns about their medicines.
  • Answer relevant medicine-related inquiries from GPs, other practice staff, other healthcare teams, and patients with queries about medicines.
  • Reconcile medicines following discharge from hospitals, intermediate care, and into care homes, identifying and rectifying unexplained changes and working with patients and community pharmacists to ensure patients receive the required medicines post-discharge.
  • Set up and manage systems to ensure continuity of medicines supply to high-risk groups of patients.
RequirementsQualifications and Experience

To be considered for this role, you must have:

  • Mandatory registration with the General Pharmaceutical Council.
  • A Master's Degree in Pharmacy (MPharm) or equivalent qualification.
  • Membership of the Royal Pharmaceutical Society.
  • Demonstrated ability to communicate complex information effectively to various audiences.
  • Capacity to plan, manage, monitor, advise, and review general medicine optimization issues in core areas for long-term conditions.
  • Good general computer skills, including Microsoft Office, email, and internet proficiency.
  • An excellent standard of English grammar and spelling.
  • A conscientious approach and adaptability in a dynamic environment.
Estimated Salary: £45,000 - £55,000 per annum
