As a RSCN Team Lead with Leaders in Care, you will be responsible for designing and delivering tailored care plans for children and their families. You will work closely with community teams, CHC nurses, and CCG commissioners to establish and maintain bespoke packages of care.

Responsibilities:
  • Develop and implement care plans that meet the unique needs of each client
  • Effectively manage multidisciplinary teams through strong communication and teamwork
  • Provide ongoing monitoring, support, and assessment of clients' physical and physiological needs
  • Deliver nursing interventions, including medication management
  • BUILD AND MAINTAIN RELATIONSHIPS WITH CLIENTS' FAMILIES

This role offers a competitive salary of up to £50,000 per annum (DOE) and a range of benefits including a £3,000 car allowance, 2 days of remote work per week, and 33 days of annual leave.
