Job Description

The Forward Trust is a leading provider of services for individuals experiencing mental health and substance misuse issues. We are currently seeking a dedicated Wellbeing Services Practitioner to join our team at HMP Chelmsford.

About the Role

  • Carry out comprehensive assessments to identify individual needs and risks.
  • Deliver evidence-based interventions to support mental wellbeing treatment journeys.
  • Develop and maintain accurate care plans, reflecting changing client needs and risks.

Key Responsibilities

  • Manage a caseload of clients, providing regular check-ins and support.
  • Collaborate with multidisciplinary teams to ensure effective service delivery.
  • Participate in regular service audits to identify areas for improvement.
