About the Role
- Carry out comprehensive assessments to identify individual needs and risks.
- Deliver evidence-based interventions to support mental wellbeing treatment journeys.
- Develop and maintain accurate care plans, reflecting changing client needs and risks.
Key Responsibilities
- Manage a caseload of clients, providing regular check-ins and support.
- Collaborate with multidisciplinary teams to ensure effective service delivery.
- Participate in regular service audits to identify areas for improvement.
