Job Title: Advanced Practice Nurse

We are seeking an experienced and skilled Advanced Practice Nurse to join our team at NHS. The successful candidate will be responsible for delivering high-quality patient care, working autonomously, and contributing to the development of our practice.

Main Responsibilities:
  • Deliver practice nursing services, working as part of the practice multidisciplinary team.
  • Provide care within their scope of practice to the entitled patient population.
  • Support the management team in reviewing clinical policy and procedure.
  • Manage clinical areas such as health promotion, chronic disease management, and health prevention.
  • Work collaboratively with other healthcare professionals to ensure seamless patient care.
Requirements:
  • Registered Nurse qualification.
  • Postgraduate diploma or degree in Advanced Practice.
  • Qualified Nurse Prescriber and Triage Nurse.
  • Teaching Qualification and ALS/PALS certification.
  • Experience working in a primary care environment, chronic disease management, and prescribing.
Clinical Knowledge:
  • Wound care, removal of sutures and staples.
  • ECGs, venepuncture, and new patient medicals.
  • Chaperone procedure, requesting pathology tests, and processing results.
  • Diabetes, asthma, CHD, and immunisations.
  • Understanding of evidence-based practice, clinical governance, and health promotion strategies.
