Mental Health Improvement Specialist

We are seeking a dynamic and motivated Band 4 Health Improvement Worker to join our team in the Gedling area. This role involves working alongside mental health teams and general practices to proactively identify and manage cardio-metabolic risk factors associated with serious mental illness.
The successful candidate will have a strong understanding of the impact of serious mental illness on patients' physical health and be able to develop and implement strategies to improve patient outcomes.
As a member of our team, you will work collaboratively with other healthcare professionals to provide high-quality care to patients in the Gedling area. You will also contribute to the development of local services and initiatives to promote the health and wellbeing of patients with serious mental illness.
Key Responsibilities:
- Work with mental health teams and general practices to identify and manage cardio-metabolic risk factors associated with serious mental illness.
- Develop and implement strategies to improve patient outcomes.
- Collaborate with other healthcare professionals to provide high-quality care to patients.
- Contribute to the development of local services and initiatives to promote the health and wellbeing of patients.
Requirements:
- Degree in a relevant field (e.g., psychology, nursing, public health).
- Experience working in a mental health setting.
- Strong understanding of the impact of serious mental illness on patients' physical health.
- Excellent communication and interpersonal skills.
