Support Worker in Independent Living

Job Role
We are looking for a dedicated and caring Support Worker to join our Care in the Home division in Hawarden. As a Support Worker, you will be responsible for supporting clients with spinal cord injury, neurological and acquire brain injury to live independently in their own homes.
Key Responsibilities
- Assist clients to exercise choice and control over their own lives
- Liaise with family members and other healthcare professionals as required
- Contribute to the development and implementation of individualised care plans
- Monitor and record client progress and identify areas for improvement
- Maintain accurate and confidential records
Working Arrangements
2 to 5 Waking Night shifts per week, Shifts 10pm -7am, Regular consistent work with set rolling rotas, Zero hours contracts, Commit to working regularly each month to enable continuity of care for our clients
