**Job Title:** Adults Disability Social Worker
We are seeking a highly motivated and experienced social worker to join our team in Chester and Ellesmere Port. As a Disability Support Specialist, you will play a vital role in providing tailored support and ensuring adults with learning disabilities can live independently.
Responsibilities:- Assessments: Conduct comprehensive assessments to identify the needs and strengths of adults with learning disabilities.
- Care Planning: Develop and implement person-centered care plans to promote independence and well-being.
- Duty Rota: Support the team's duty rota, responding promptly to urgent cases and ensuring continuity of care.
- Safeguarding: Apply relevant legislation, including the Care Act, Mental Capacity Act, and safeguarding policies, to uphold the rights and safety of service users.
- Record Keeping: Maintain accurate and timely records in line with council policies and procedures.
The estimated salary for this role is £32,000 - £40,000 per annum, depending on experience.
