About the Role

We are looking for enthusiastic Social Care Assessors who have an understanding of adult social care, mental capacity, and safeguarding issues. The ideal candidate will be self-motivated with excellent communication skills and the ability to develop and work both autonomously within Adult Community Services and the wider multi-disciplinary arena.

Key Responsibilities

  • Conduct thorough assessments of individuals to identify their needs and develop personalized support plans.
  • Work collaboratively with other professionals, including healthcare providers, social workers, and community organizations, to ensure seamless service delivery.
  • Provide guidance and support to individuals, families, and carers to empower them to make informed decisions about their care and well-being.
  • Develop and maintain accurate and comprehensive client documentation, adhering to Council policies and professional practice.
  • Participate in training and professional development opportunities to enhance skills and knowledge in adult social care.

Requirements

  • Relevant health and social care qualification and experience in adult social care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Ability to work autonomously and as part of a team, with a flexible approach to work and a willingness to adapt to changing circumstances.
  • Good organizational and time management skills, with the ability to prioritize tasks and manage multiple cases.
  • Understanding of asset and strengths-based support, with the ability to remain calm and composed in challenging situations.
