Cheshire East Council is seeking self-motivated Social Care Assessors to join our innovative and friendly Adult Social Care Teams. These are temporary posts initially for a period of 12 months and will be reviewed as required.
About the Role
We are looking for enthusiastic Social Care Assessors who have an understanding of adult social care, mental capacity, and safeguarding issues. The ideal candidate will be self-motivated with excellent communication skills and the ability to develop and work both autonomously within Adult Community Services and the wider multi-disciplinary arena.
Key Responsibilities
- Conduct thorough assessments of individuals to identify their needs and develop personalized support plans.
- Work collaboratively with other professionals, including healthcare providers, social workers, and community organizations, to ensure seamless service delivery.
- Provide guidance and support to individuals, families, and carers to empower them to make informed decisions about their care and well-being.
- Develop and maintain accurate and comprehensive client documentation, adhering to Council policies and professional practice.
- Participate in training and professional development opportunities to enhance skills and knowledge in adult social care.
Requirements
- Relevant health and social care qualification and experience in adult social care.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
- Ability to work autonomously and as part of a team, with a flexible approach to work and a willingness to adapt to changing circumstances.
- Good organizational and time management skills, with the ability to prioritize tasks and manage multiple cases.
- Understanding of asset and strengths-based support, with the ability to remain calm and composed in challenging situations.
