The Role

We are seeking an enthusiastic and motivated Care Professional - Elderly Support Specialist to join our friendly team. As a key member, you will play a vital role in supporting individuals to maintain their independence, engage in local community activities, and achieve their personal goals.

Responsibilities

  • Develop Person-Centred Plans in collaboration with service users, their families, and multidisciplinary teams.
  • Provide person-centred support to promote independence, confidence, and participation in community activities.
  • Maintain accurate records of service user progress, noting successes and areas for development.

Requirements

  • Degree or equivalent experience in social care or a related field.
  • Possess excellent communication, interpersonal, and organisational skills.
  • Ability to work collaboratively as part of a team and independently.

Salary and Working Arrangements

The salary for this position is approximately £24,000-£30,000 per annum (pro rata), depending on experience. This is a full-time role, however, we welcome applications from candidates interested in flexible working arrangements.
