We are seeking a highly skilled Care Coordinator to join our team at Carers Direct Ltd. As a key member of our staff, you will play a vital role in ensuring that our service users receive the highest level of care and support.

About the Role:

  • Serve as a pivotal point of contact for our service users, providing them with guidance, support, and reassurance when needed.
  • Work closely with our care staff to develop and implement personalized care plans that cater to each individual's unique needs.
  • Collaborate with other departments within the organization to ensure seamless communication and coordination.
  • Maintain accurate records and reports on all aspects of care delivery, including staff performance and service user feedback.
  • Contribute to the development and implementation of new policies and procedures to improve overall quality of care.

Requirements:

  • Degree in Social Work or related field (or equivalent experience)
  • Minimum 2 years' experience in care management or related field
  • Excellent interpersonal and communication skills
  • Ability to work independently and as part of a team
  • Strong problem-solving and analytical skills
  • Basic IT skills, including Microsoft Office and email software
